Patient Information

We would like to take this opportunity to welcome you to our practice. My favorite part about practicing dentistry is getting to know my patients on a personal level. When I began practicing with Dr. Robert Heacox I was inspired by his sincere interest and desire to get to know his patients and the respect he had for his staff. It's clear now why his practice has three, and even four, generations of family members as patients and staff members who have been with him for 15 to 20 years. I hope, as our patient, you will feel comfortable and welcomed when you enter our office and we look forward getting to know you.

Your First Initial Appointment

As a new patient to our practice, being seen for a routine appointment, please allow 2 hours of time. At this appointment we will collect and review your medical and dental history. We will take appropriate xrays and do a thorough exam which includes:

Periodontal Examination
Head and Neck Examination
Intraoral Examination - oral cancer screening, decay detection, teeth wear, teeth function and alignment of teeth.
Teeth Cleaning (please note additional time for a cleaning may be needed and could be scheduled at another date).
Treatment Plan

For your convenience you can print patient information forms from our website. You can mail, email, fax or simply bring your forms to your appointment with you.

Filling out patient information forms at the time of your appointment is fine too.

Cancellations

We kindly request a 24 hour notice to cancel or change an appointment. However, we understand things come up. We are reachable by phone (253) 851-6771 for any appointment changes. Thank you!

Financial Policy

We are contracted with some of the large dental insurance carriers in our area. We are happy to file insurance forms on your behalf and we try to estimate your benefits as closely as we can. However, please know that your insurance policy is an agreement between you and the insurance company. We will work very hard to utilize your insurance so you get the most benefits from your plan. Patients are responsible for balances not paid for by insurance.

In addition, we offer CareCredit at 6 months interest free terms.

If you don’t have insurance that is no problem at all.We offer a 10% courtesy discount on treatment that is paid in full at the time of service. Please note, a 10% discount will not be given when balance is paid with CareCredit.
